Authorization

I authorize the SER to videotape and audiotape my complete presentation(s) and consent to allow the use and reproduction of my name, presentation, and all materials contained in my presentation, including handout materials (except the material listed below).  My videotaped and audiotaped presentation and materials will only be used, reproduced, and made available to the SER members through the SER webpage or communications.

I acknowledge the benefits associated with having my presentation included in the video/audio program and that a copy of my presentation(s) will be given to me upon request.  I understand and agree that this is the entire consideration and compensation due to me for the use, reproduction or distribution of the Conference.

To the best of my knowledge, use, and reproduction of the materials or information used in my presentation(s) will not violate any third party’s copyrights or other property rights.  To the extent copyrighted or trade secret materials re used, reproduced, or displayed with my presentation, I have either obtained written permission to used, reproduce, and distribute such materials from the copyright owner.
